Application Process

1. The candidate is required tofill an online applicationformavailable at UHS website www.uhs.edu.pk
2. Before applying online, the candidate must deposit a process fee of Rs.1000/-(Non-refundable) through Bank Challan at National Bank of Pakistan, Sheikh Zayed Hospital Branch Lahore (A/C # 3164498127) or get a Pay Order/Demand Draft in the name of Treasurer, Universityof Health Sciences, Lahore. The candidate shall enter the number of his/her bank challan/DD/PO in his/her online application form.
3. The applicantmust fill the forms carefully. He/she is required to complete the application by entering correct information. After completely filling the application, the candidate must review the information entered by him/her. Before submission, the candidate shall be able to edit his/herinformation. Once the application is submitted, the candidate shall no tbe allowed to edit it.
4. The candidate should get a print of the online filled application form by clicking "Download & Print" button. If the facility of downloading and printing is not available at that time, the candidate can access his/her application any other time by clicking Search Button.
5. The print out of filled applicationform, alongwith the attested copies of certificates, testimonials, CNIC, Original fee deposit receipt/DD/PO, and passport size colored photographs,must reach the office of the Director Admin & Coord, University of Health Sciences, Khayaban-e-Jamia Punjab, Lahore-Pakistan, by post or by hand, on or before 25 th October, 2020 by 5:00 PM.
6. It shall be mandatory for the candidate to complete both phases of Application Submission, i.e., online and manual. Mere submission of online applicationshall not be considered valid for candidature.

Interview	5th to 6th November, 2020

Fee Submission	by 13th November, 2020
Final Merit List	16th November, 2020
Commencement of Classes	23rd November, 2020

Note

1. Applicationsreceived after due date or with incomplete information shall be rejected
2. Admission shall be granted onopen meritbasisonly.
3. Those in service and applicants desirous to attend the course on deputation basis must apply through proper channel.
4. Those having foreign qualifications are required to submit HEC/PMDC/PMC equivalence certificate
